In atrial fibrillation, the greatest danger is stroke. Blood pools in a small pouch off the left atrium called the left atrial appendage, where clots can form and then travel to the brain — and the large majority of stroke-causing clots in non-valvular atrial fibrillation originate there.

Blood thinners prevent those strokes effectively and remain first-line therapy. But some people cannot take them safely. The Watchman FLX device closes the appendage instead: a small, expandable implant is placed at its opening, and over the following weeks the body's own tissue grows over the surface, sealing the pouch off from the circulation.

Watchman is for patients with non-valvular atrial fibrillation at meaningful stroke risk who have a reason not to remain on long-term anticoagulation — most often serious or recurrent bleeding, a bleeding-prone condition, or a high risk of falls or trauma.

It is not for everyone with atrial fibrillation. If you tolerate a blood thinner well, that remains the better-established therapy. Implantation also requires the ability to take anticoagulation and antiplatelet medication for a period after the procedure while the device heals over, which is a common point of confusion — the device does not eliminate medication immediately.

It does not treat the arrhythmia itself. Your heart will still be in atrial fibrillation, and rate or rhythm control continues unchanged.

Watchman is implanted in a catheterization lab under general anesthesia, and typically takes about an hour. A catheter is advanced from a vein in the groin to the right atrium and then across the wall between the atria into the left atrium. Guided by imaging — a transesophageal echocardiogram or intracardiac ultrasound, along with X-ray — the device is positioned at the mouth of the appendage, expanded, and tested for stability and seal before release.

Most patients stay one night and go home the next day. The groin site needs care for several days, with no heavy lifting or strenuous activity for about a week.

You will be on medication for a defined period while tissue grows over the device — commonly an anticoagulant or an antiplatelet combination for roughly 45 days, then a period on aspirin and clopidogrel, then typically aspirin alone. The exact regimen varies and is set by your cardiologist.

A follow-up imaging study at around 45 days confirms the appendage is sealed and no clot has formed on the device. That study is what permits the blood thinner to be stopped, so keeping the appointment matters more than most follow-ups.

Tell any clinician you have the implant, and ask about antibiotics before dental or surgical procedures.

The main procedural risks are bleeding or vascular injury at the groin, fluid around the heart requiring drainage, device movement, clot formation on the device surface, and — uncommonly — stroke during the procedure itself. A residual leak around the edge of the device is sometimes seen and is usually followed rather than treated. Your cardiologist will discuss how these apply to you against the bleeding risk that prompted considering the device in the first place.
