Aterectomía

La aterectomía elimina o modifica la placa endurecida dentro de una arteria para que un balón y un stent puedan abrirla correctamente; se utiliza cuando una obstrucción está demasiado calcificada.

Acerca de Aterectomía

Some blockages are too hard to push aside. When plaque has become heavily calcified, a balloon may not expand it — or may expand it unevenly, leaving a stent underdeployed, which is a well-recognized cause of later stent failure. Atherectomy addresses this by removing or modifying the calcium first, preparing the vessel so that angioplasty and stenting can work as intended.

It is not a routine part of every intervention. It is a specialized technique for a specific problem, and it is used when imaging or the behavior of the balloon shows that calcium is the obstacle.

The techniques

  • Rotational atherectomy — a small diamond-coated burr spins at high speed and abrades calcified plaque into particles small enough to pass harmlessly through the circulation
  • Orbital atherectomy — an eccentrically mounted crown orbits against the plaque, sanding calcium away while remaining gentler on healthy, elastic vessel wall
  • Laser atherectomy — pulsed ultraviolet energy vaporizes plaque, useful in certain lesions and inside a failed stent
  • Directional atherectomy — a blade shaves plaque and captures it in the catheter, used more often in peripheral arteries than coronary ones
  • Intravascular lithotripsy — not strictly atherectomy, but related in purpose: sound pressure waves crack calcium within the vessel wall, much as lithotripsy treats a kidney stone

Which is chosen depends on the lesion, and intravascular imaging with ultrasound or OCT is often used to see the calcium directly and guide the decision.

What to expect

Atherectomy is performed during a cardiac catheterization, so the experience is the same: wrist or groin access, sedation while you remain awake, and a procedure that usually runs somewhat longer than a standard angioplasty. It is nearly always followed by balloon angioplasty and stenting in the same session — atherectomy prepares the vessel rather than treating it alone.

You may feel chest discomfort during treatment, and some patients notice a sensation during rotational atherectomy runs. The team monitors continuously and works in short bursts.

Recovery, medication and follow-up match angioplasty and stenting, including dual antiplatelet therapy, which must not be stopped without speaking to your cardiologist.

Risks

In addition to the usual risks of catheterization, atherectomy carries a somewhat higher chance of vessel injury or perforation, slow flow in the treated artery, and dislodged particles traveling downstream. In experienced hands these are uncommon, and the alternative — placing a stent into a rigid, calcified segment that will not fully expand — carries its own significant long-term risk. Your interventional cardiologist will explain why the technique was chosen for your anatomy.
