Cirugía de bypass
La cirugía de bypass periférico desvía la sangre alrededor de una arteria obstruida mediante un injerto de vena o sintético, y restablece la circulación de una pierna amenazada por una obstrucción grave.
Acerca de Cirugía de bypass
When an artery in the leg is blocked over a long segment, or is too calcified to reopen from within, blood can be routed around it instead. A bypass graft is sewn to the artery above the blockage and again below it, creating a new channel — the obstructed segment is left in place and simply bypassed.
Bypass is the more invasive option compared with angioplasty and stenting, and it is chosen when durability matters most or when the pattern of disease makes an endovascular approach unlikely to last.
Why it is done
- Chronic limb-threatening ischemia — rest pain, non-healing wounds or gangrene, where circulation must be restored to save the limb
- Disabling claudication that has not responded to a supervised exercise program and medical therapy
- A long, heavily calcified or totally occluded segment unsuited to stenting
- A previous endovascular treatment that has failed
The graft
Where possible your own vein is used, most often the greater saphenous vein from the leg. Vein grafts last longer than synthetic ones, particularly for bypasses below the knee, and mapping the vein with ultrasound beforehand is part of planning. When no suitable vein is available, a synthetic graft is used.
Common configurations are named for what they connect — femoral-popliteal, femoral-tibial, aortobifemoral — and the choice follows your anatomy.
What to expect
Bypass is performed under general or regional anesthesia and can take several hours. Incisions are made to expose the artery above and below the blockage, and to harvest the vein if one is used. Blood flow is temporarily clamped while the graft is sewn into place, then restored and checked.
Expect a hospital stay of several days. Circulation to the foot, wound healing and pain control are the focus, and walking begins early with assistance.
Recovery
Recovery is longer than for a catheter-based procedure — commonly several weeks before normal activity, more if a wound is healing.
- Leg swelling on the operated side is very common and can last months; elevation and the compression your team recommends help
- Wound care matters, and incisions in the groin and along the leg are watched closely
- Numbness along an incision is common
- Walking is encouraged progressively; heavy lifting and driving wait until cleared
Antiplatelet medication, and sometimes an anticoagulant, is prescribed to keep the graft open, along with a statin. Stopping smoking is the single most important thing you can do — continued smoking is strongly associated with graft failure and limb loss.
Surveillance
Grafts are followed with ultrasound surveillance on a schedule, typically beginning within the first month and continuing at intervals for years. The purpose is to find a narrowing while the graft is still open, because a narrowing can be repaired, whereas a graft that has closed usually cannot be salvaged. These appointments are not routine formalities.
When to seek care urgently
Sudden coldness, numbness, pallor or severe pain in the leg or foot may mean the graft has closed and requires emergency evaluation. Also call for fever, spreading redness, wound drainage, or bleeding.
Risks
Bleeding, wound infection, graft failure or clotting, heart or lung complications from the anesthesia and the stress of surgery, swelling, and — in the most severe disease — the possibility that the limb cannot be saved. Your vascular surgeon will discuss how these apply to your circulation, wounds and overall health.
