Who qualifies for Chronic Care Management?

The program is intended for patients with two or more chronic conditions expected to last at least twelve months, such as coronary artery disease, heart failure, hypertension, atrial fibrillation, diabetes, or high cholesterol. Enrollment is voluntary and requires your consent.

What happens between my office visits?

Your care team checks in regularly by phone, telemedicine, or in person to track your progress, review medications, and address new symptoms. Many patients also use remote monitoring devices such as blood pressure cuffs or scales so changes are noticed early.
